Intelligent Outreach at Scale: CRM-driven Models for Preventive Health

Abstract The wide implementation of preventive healthcare requires solutions to deliver individualized outreach programs at large scales for building sustainable health systems. Current healthcare methods require significant resources, yet they do not effectively overcome the social and organizational obstacles that prevent patients from participating. This paper shows that integrating cloud computing with CRM principles and AI technology creates a revolutionary solution. The proposed model uses Salesforce Health Cloud to merge various data sources, including EHRs and patient-generated data, into one comprehensive patient profile. The patient profile supports two intelligent outreach methods: 1) Salesforce Einstein uses ML for predictive risk stratification to identify at-risk patients before they need intervention, and 2) Salesforce Marketing Cloud enables automated personalized multi-channel communication that addresses patient needs and barriers. The model works to enhance patient adherence rates and health results while optimizing operational performance. The paper evaluates multiple obstacles, including expenses, system integration issues, privacy concerns, HIPAA requirements, and algorithmic bias problems. The paper demonstrates that CRM-based models show promise for preventive care, but their equitable success requires robust data governance together with ethical oversight.
