Abstract Blood cancers, including leukemia, lymphoma, and multiple myeloma, represent a significant global health burden despite advances in diagnosis and treatment. Limitations associated with conventional therapies, such as drug resistance, systemic toxicity, and high treatment costs, have driven growing interest in plant-based therapeutics as complementary or alternative approaches. This review explores the potential of medicinal herbs in blood cancer management by integrating ethnobotanical knowledge with contemporary oncological research. Traditional medical systems such as Ayurveda, Traditional Chinese Medicine, and Indigenous healing practices have long employed medicinal plants for blood-related disorders, providing valuable leads for modern drug discovery. The review highlights key medicinal herbs with demonstrated anti-blood cancer activity and summarizes preclinical and emerging clinical evidence supporting their efficacy. Mechanistically, plant-derived bioactive compounds exert anticancer effects through multiple pathways, including induction of apoptosis, cell cycle arrest, modulation of oncogenic signaling pathways, immune regulation, and epigenetic modification. Additionally, the review discusses safety considerations, toxicity profiles, and potential herb-drug interactions relevant to integrative oncology. Despite promising findings, challenges related to standardization, bioavailability, regulatory oversight, and limited clinical validation remain major barriers to clinical translation. Overall, plant-based therapeutics hold significant promise as adjunct strategies in blood cancer treatment. Rigorous clinical trials, standardized formulations, and interdisciplinary research approaches are essential to translate traditional medicinal knowledge into evidence-based therapeutic applications for hematological malignancies.
